Autoregulation and heart rate.

چکیده

A utoregulation is an important physiological mechanism whereby blood flow to an organ is kept relatively constant as perfusing pressure changes. For all organs except the heart, autoregulation can be examined by changing aortic pressure and measuring the resulting organ flow. In the left ventricle, however, changing aortic pressure alters myocardial oxygen consumption, which itself alters myocardial blood flow. Therefore, autoregulation in the left ventricle can be tested only by cannulating the left coronary artery so that coronary perfusing pressure can be changed while aortic pressure (and myocardial oxygen consumption) remain constant. If this is done and coronary perfusing pressure is suddenly changed, coronary blood flow initially changes transiently in the direction of the pressure change (decreasing when pressure is lowered and increasing when pressure is raised) and then returns towards its control value.1-5 Autoregulation takes place in all layers of the left ventricle, thus preserving the approximate equality of flows per gram across the ventricular wall as coronary perfusing pressures change.6,7 There is a limit to how low coronary perfusing pressure can decrease before autoregulation fails to keep coronary flow relatively constant. This limit is

Myocardial oxygen tension determines the degree and pressure range of coronary autoregulation.

Experiments were designed to separate effects of myocardial oxygen tension and oxygen consumption on coronary autoregulation. The approach was to measure coronary hemodynamic and metabolic responses to decreases in perfusion pressure during interventions that altered the balance between myocardial oxygen supply and demand. Studies were conducted in anesthetized heart-blocked dogs with the left ...

Coronary blood flow autoregulation and flow heterogeneity in the stunned heart.

We used an anesthetized swine model of regionally "stunned" myocardium to determine the effect of stunning on coronary autoregulation and blood flow heterogeneity. In 18 domestic swine, stunning was accomplished by reducing blood flow to the left anterior descending coronary artery (LAD) by approximately 75% of baseline for 15 min and restoring it to normal for 1 hour. We quantified coronary au...

Autoregulation of cardiac output is overcome by adrenergic stimulation in the anaconda heart.

Most vertebrates increase cardiac output during activity by elevating heart rate with relatively stable stroke volume. However, several studies have demonstrated 'intrinsic autoregulation' of cardiac output where artificially increased heart rate is associated with decreased stroke volume, leaving cardiac output unchanged. We explored the capacity of noradrenaline to overcome autoregulation in ...

Cerebral autoregulation is preserved in postural tachycardia syndrome.

To test whether cerebral autoregulation is impaired in patients with postural tachycardia syndrome (POTS), we evaluated 17 healthy control subjects and 27 patients with POTS. Blood pressure, heart rate, and cerebral blood velocity (transcranial Doppler) were recorded at rest and during 80 degree head-up tilt (HUT). Static cerebral autoregulation, as assessed from the change in cerebrovascular r...

Dynamic cerebral autoregulation is preserved in neurally mediated syncope.

To test whether cerebral autoregulation is impaired in patients with neurally mediated syncope (NMS), we evaluated 15 normal subjects and 37 patients with recurrent NMS. Blood pressure (BP), heart rate, and cerebral blood velocity (CBV) (transcranial Doppler) were recorded at rest and during 80 degrees head-up tilt (HUT). Static cerebral autoregulation as assessed from the change in cerebrovasc...
